You must download and complete the following 5 forms and email them to office@cvpkids.com or fax them to (860) 355-3856 before your first visit. (We’d prefer that you send them in before you pre-register).

- New Patient Contract
- Medical Records Release Form
- Demographic Sheet
- Waiver
- Credit Card Form

You must also read and agree to the Vaccine Policy.
