  • is delivered through a personal/primary care provider. At Candlewood Valley Pediatrics, a physician leads a care team dedicated to the ongoing care of the patient and his/her family.

  • enhances access to timely care, coordinated and/or integrated across healthcare systems.

Medical Home Responsibilities

In our patient centered medical home, you, your child, the pediatrician, and our highly trained staff work together as a team. With this approach, we are able to provide your family with superior health care.

  • You may contact your Medical Home Team directly to arrange an appointment or to discuss your healthcare needs by calling 860-355-8190 Monday through Thursday between the hours of 8:00 am and 5:45 pm, or Friday 8:00 am to 4:30 pm, or Saturday 8:00 am to 12:00 pm
  • Outside the normal office hours for any urgent matters, you may call 860-355-8190 and the on-call provider will call you back.
  • Please continue to use 911 for all emergencies.
  • Please make a list of your health questions.
  • Please make a list of specialists you have visited since your last appointment; include their names, addresses, phone numbers, and the reasons you visited them.
  • Please take all your medicines in their original containers to your appointment. Be sure to include prescription, over-the-counter, natural, and herbal medicines and vitamin supplements.
  • Please take your insurance card or other insurance information with you to your appointment.
  • If it is a first-time visit, the child must have a parent or guardian present.
  • Please be sure you know what you should do before you leave the office: make further appointments, receive school/sport/camp forms, take with you all handouts the office staff has provided you with.
  • Please inform any other doctor you see that your primary care provider is Candlewood Valley Pediatrics and request that they provide us with a report of your visit.

Candlewood Valley Pediatrics requests that patients have all other medical providers forward reports of visits to our office. When patients inform our staff of visits for which we have not received reports, the receptionist for the primary doctor will send a written request to that office/lab/radiology center/etc. to have the information forwarded to CVP. The information will be included in the patient’s electronic chart.

Should a patient require a visit to another care provider for which a referral is necessary, the CVP Referral Specialist will be informed and will tend to that referral.
