Candlewood Valley Pediatrics was founded in 1988 by the husband and wife team of Evan Hack, MD, and Diane D'Isidori, MD. While we have grown as a practice, our core beliefs have never changed. We are committed to providing quality, family-oriented care in a friendly and compassionate environment. Each child we touch is unique and deserving of individualized care and respect. We are a team that is dedicated to going beyond what is expected to enhance your family's well being. Throughout our 25-year history, Candlewood Valley Pediatrics has continuously strived to provide comprehensive care of the highest quality to our patients and their families. To achieve these goals most effectively, the practice uses an ongoing quality improvement process to set goals and act to improve performance. We are a fully certified PCMH practice.*
Our practice provides preventative care as well as the diagnosis and treatment of the full spectrum of illness in infants, children and adolescents.
Our medical practice consists of seven full-time practitioners (five physicians and two PAs). All physicians are on staff at Nuvance Health (formerly known at Western Connecticut Health Network) and Connecticut Children’s Medical Center (CCMC) in Hartford.

Office Manager
The CVP Office Manager is available to speak to Monday through Friday from 8:30 am to 4:30 pm. Should you have any issues with policies, procedures, or any other office staff, please contact the Office Manager to discuss these issues.
Melanie and Inga
Patient Care Coordinators
The CVP Patient Care Coordinators are available to speak with you Monday through Friday from 8 am to 4:30 pm. They will help you with specialist referrals and prior authorizations.
Noreen McPartland
Billing/Insurance Coordinator
CVP staff is available to help you with billing and insurance issues Monday through Friday from 8 am to 4:30 pm. If the front staff is unable to help, your issue will be handled by the CVP billing/insurance coordinator.
Nursing Staff
There are several nurses on staff who are available to speak with patients throughout the day. If you have a question about your child’s checkup needs, immunization needs, or acute illness issues, please ask to speak to one of the nurses. If the nurse is not able to answer your question adequately, she will pass it along to your child’s provider.
Reception Staff
There are several receptionists on staff to answer the phones Monday through Thursday from 8:00 am to 5:45 pm, Friday from 8:00 am to 4:30 pm, and Saturday from 8:00 am to 12 pm. The receptionists are available to make appointments, take messages, pass along questions to doctors and nurses, and to answer general questions about office visits, checkup visits, and health forms.

*A Patient Centered Medical Home (PCMH) is a team-based model of care led by your personal physician. The team provides continuous and coordinated care to maximize outcomes while working to decrease the overall cost of healthcare. A PCMH is not a building, house or hospital but rather an approach to providing healthcare services in a high-quality, cost effective manner (American Academy of Pediatrics, 2004). A PCMH is responsible for providing and coordinating your healthcare needs by arranging consultations with and continuously communicating with other qualified healthcare professionals involved in your care. This also includes the provision of preventive services, treatment of acute and chronic illnesses and assisting with the transition of care. "Patient Centered" means YOU are the most important person on your healthcare team.
