Please carefully read all of the following links before filling out the Pre-Registration form. Some of the forms must be filled out and returned to the office by email (office@cvpkids.com) or fax (860-355-3856) prior to making an appointment.
Your child will not be scheduled for an appointment until you have completed and returned to our office these five forms AND a copy of your child's immunizations.
